Locations may vary

Q: What do I need to bring to get a vaccine when it’s my turn?
When you sign up for the vaccine, there will be documentation available about what you need to bring.  Please bring ID if you have it, your insurance card, and your FNRC letter of disability.  Some clinics will have a consent form that will need to be completed as well as a prevaccination check-list.   If you have questions, please call your service coordinator.
